Sports physical appointments for the 2021-2022 school year can be made at the Sanford Mayville Clinic. Student athletes grades 7-12 can contact the clinic Monday through Friday during clinic hours 8am-4:30pm to set up an appointment. Appointments are required. Please complete the NDHSAA Physical Evaluation Form and bring it with you to the appointment. Forms can be found online at https://ndhsaa.com.
